When you are downed you currently have a small chance to stand up again.
Make it so you have a higher chance of standing up on your own when you are on good food/water conditions!
This would add value to food and may help solos.
I honestly think you should stand up regardless of food/water and the only thing that prevents you getting up are environment effects like being too cold or getting downed on fire. I never understood why this should be a rng mechanic. It takes one bullet/arrow/melee hit to finish of a downed player, so it should be the responsibility of the attacker to finish you off. I would even go a step further and say they should add a downed system in games like PUBG where you can crawl away to a safer spot so a friend can actually revive you in a otherwise unwinnable situation.

Agree, encourgae players to have to finish you to take you out of the fight.
It should work on bleedout only.
Could be a reward for living longer, more survivability when downed and more hp threshold for being downed, less chance of instagib
I would add a small minigame, like QTE that becomes easier when you have your needs up, but becomes really hard when you are low on food and drink.

In any of those cases, you sould be able to use med items
There’s only such a hard injury that would render you unable to syringe or bandage yourself.
-note that, this should not get you instant-up, only help health regen, better improving the odds of getting back up… So, having a healing item on you, on those cases, would be worth.
Not be able to craft anything, or eat tho, just specificly med items
At the very least, have it so that the longer you wait the more chance you have. It’s fucking horrible to have waited the 30seconds just in case hwile your mates are in a fight as a strategic decision instead of just /killing immediatly only to finally die on the 29th second. If each time it ‘ticked’ you increased chance to res, you might actually feel it was more ‘just’.
Also improve the damn animations, what happened to the player rolling over on his stomach and holding his back after he got downed, what happened to the player falling down on his back and holding his stomach after his was downed screaming in pain? It is sad to see all these things that made Rust, Rust dissapear. Also when you would get picked up your player actually stood up by using his feet and hands, Rather than him instantly just going from down to up in a mili second. What about a person shaking his head after he gets headshot. We need these animations back.
